FDA Approves Voyxact to Reduce Proteinuria in Adults With IgA Nephropathy

What Was Approved?

On November 25, 2025, the U.S. Food and Drug Administration approved Voyxact (sibeprenlimab-szsi) to reduce proteinuria in adults with primary immunoglobulin A nephropathy, or IgAN, who are at risk for disease progression.

The approval was granted through the FDA’s accelerated approval pathway.

How Does Voyxact Work?

Voyxact is an APRIL blocker.

APRIL is an immune-system signaling protein involved in antibody production. Blocking this pathway is intended to reduce disease-related immune activity involved in IgA nephropathy.

How Is Voyxact Given?

The recommended dose is:

400 mg by subcutaneous injection once every 4 weeks.

What Did the Clinical Trial Show?

Voyxact was studied in the randomized, double-blind, placebo-controlled VISIONARY trial.

At Month 9, patients receiving Voyxact had an average 50% reduction in urine protein from baseline, compared with a slight increase in the placebo group. The relative reduction compared with placebo was about 51%.

These results describe the study population and do not guarantee the same response for every patient.

Does Voyxact Provenly Slow Kidney Disease Progression?

Not yet.

This is an important limitation of the approval.

FDA states that it has not yet been established whether Voyxact slows long-term decline in kidney function. Continued approval may depend on a confirmatory clinical trial showing clinical benefit.

What Are the Common Side Effects?

The most common adverse reactions in the clinical trial were:

  • Infections
  • Upper respiratory tract infections
  • Injection-site reactions
  • Injection-site redness

Because Voyxact affects immune-system activity, patients should be monitored appropriately during treatment.

Bottom Line

Voyxact (sibeprenlimab-szsi) is an FDA-approved once-every-four-weeks injectable treatment for adults with primary IgA nephropathy who are at risk for disease progression.

It was approved based on its ability to reduce proteinuria, but long-term preservation of kidney function has not yet been confirmed.
