What Was Approved?
On May 1, 2026, the U.S. Food and Drug Administration approved Veppanu (vepdegestrant) for adults with:
- Estrogen receptor-positive (ER-positive) breast cancer
- HER2-negative breast cancer
- An ESR1 mutation detected by an FDA-authorized test
- Advanced or metastatic disease
- Disease progression after at least one prior line of endocrine therapy
Veppanu is not approved for every patient with ER-positive breast cancer.
How Does Veppanu Work?
Veppanu is a heterobifunctional protein degrader.
It is designed to promote degradation of the estrogen receptor, which can help interfere with estrogen-driven signaling in susceptible breast cancer cells.
How Is Veppanu Taken?
The FDA-approved recommended dose is:
200 mg by mouth once daily with food.
Dose interruption, reduction, or discontinuation may be needed if significant adverse reactions occur.
What Are the Main Safety Concerns?
The prescribing information includes warnings for:
- QTc interval prolongation
- Embryo-fetal toxicity
The FDA label recommends checking ECGs and electrolytes before treatment and correcting low potassium or magnesium when necessary.
What Are Common Side Effects?
Common adverse reactions and laboratory abnormalities included:
- Fatigue
- Musculoskeletal pain
- Nausea
- Decreased appetite
- Constipation
- Changes in blood-cell counts
- Increased liver enzymes
- Low potassium
Other adverse effects may also occur.
Are Drug Interactions Important?
Yes.
Veppanu can interact with medicines that affect CYP3A, as well as certain P-gp and UGT1A9 substrates.
Patients should provide their oncology team and pharmacist with a complete list of medications and supplements before starting treatment.
Bottom Line
Veppanu (vepdegestrant) is a newly FDA-approved oral targeted treatment for certain adults with ER-positive, HER2-negative, ESR1-mutated advanced or metastatic breast cancer after progression on prior endocrine therapy.
Because treatment eligibility depends on the presence of an ESR1 mutation, appropriate biomarker testing is required.
