FDA Approves Idvynso as a Two-Drug HIV-1 Treatment Regimen

What Was Approved?

On April 20, 2026, the U.S. Food and Drug Administration approved Idvynso (doravirine and islatravir) as a complete treatment regimen for certain adults with HIV-1 infection.

It is intended to replace a current antiretroviral regimen in adults who:

  • Are already virologically suppressed, with HIV-1 RNA below 50 copies/mL
  • Are stable on their current antiretroviral regimen
  • Have no history of virologic treatment failure
  • Have no known resistance-associated substitutions to doravirine

Idvynso is not approved as an initial treatment for every person newly diagnosed with HIV.

What Is Idvynso?

Idvynso combines two antiviral medicines:

  • Doravirine, a non-nucleoside reverse transcriptase inhibitor
  • Islatravir, a nucleoside analog reverse transcriptase inhibitor

Together, they interfere with HIV replication.

How Is Idvynso Taken?

The recommended dose is:

One tablet by mouth once daily, with or without food.

Each tablet contains 100 mg of doravirine and 0.25 mg of islatravir.

What Are the Main Safety Concerns?

The FDA-approved label includes warnings for serious skin and hypersensitivity reactions, including:

  • Stevens-Johnson syndrome
  • Toxic epidermal necrolysis
  • DRESS syndrome

Patients should seek prompt medical attention for severe rash, blistering, facial swelling, fever, or other concerning symptoms.

Are Drug Interactions Important?

Yes.

Idvynso should not be taken with certain strong CYP3A enzyme inducers, because they can lower doravirine levels and reduce treatment effectiveness.

It is also contraindicated with lamivudine or emtricitabine because these drugs may reduce exposure to the active form of islatravir.

Because Idvynso is already a complete HIV regimen, adding other antiretroviral medicines is generally not recommended unless specifically directed.

What Are Common Side Effects?

The most common adverse reactions reported in clinical studies included:

  • Diarrhea
  • Dizziness
  • Fatigue
  • Abdominal bloating
  • Headache
  • Weight gain

Other adverse effects may also occur.

Bottom Line

Idvynso (doravirine and islatravir) is a newly FDA-approved once-daily two-drug regimen for certain adults with HIV-1 who are already virologically suppressed on a stable treatment regimen.

It is a switch option rather than a general first-line treatment, and careful review of resistance history and drug interactions is important before changing therapy.
